While TPS provides employees a competitive company paid Life and AD&D Insurance policy, sometimes individuals and families need additional protection to accomplish their goals. TPS is pleased to offer additional Voluntary Life and AD&D Insurance to all eligible employees through Guardian. This is a voluntary program.
- Voluntary Life and AD&D insurance is paid by the employee.
- There are no medical questions for coverage to be issued under the guarantee issue amount when you are first eligible.
- Guarantee Issue coverage is available for employees, spouses and children.
Employee Options:
In multiples up to 5 times your base pay, to a maximum of $500,000. Guarantee issue is $100,000 at time of initial enrollment. Amounts that exceed guarantee issue require evidence of insurability.
Please note: Benefits are reduced by 35% at age 65, 60% at age 70, 75% at age 75.
Spouse Options:
You may elect one of the following benefit options: $10,000, $25,000, $50,000, $75,000, or $100,000. Guarantee issue is $25,000 at time of initial enrollment. Amounts that exceed guarantee issue require evidence of insurability.
Child Options:
10% of employee election to a maximum of $20,000. Available for children from birth to age 20, or 26 if full time student. Guarantee issue is $10,000 at time of initial enrollment. Amounts that exceed guarantee issue require evidence of insurability.
