Your coverage is provided through Guardian which offers you an extensive network of dental providers. Remember, using an in-network PPO dentist can save you money. If you use an out-of-network dentist, you can be balanced billed by your provider.

*When you receive services from a Nonparticipating Dentist, the percentages in this column indicate the portion of Guardian’s Nonparticipating Dentist Fee that will be paid for those services. The Nonparticipating Dentist Fee may be less then what the dentist charge and you are responsible for the difference.
